LITTLE STARS SPRING 2024

EXPLORE the wonders of the stage with our youngest actors and their friends. This 90-minute class teaches acting, singing and dancing fundamentals and incorporates the wonderful world of imagination that is so vibrantly alive in young children.

Little Stars is for 4-6 year olds.

Little Stars meets every Saturday from 12:15-1:45pm. In-person classes will be held at Lakewood Center for the Arts, 368 S. State St., Lake Oswego, OR 97034.

Spring Term runs from Apr 6th thru Jun 8th (includes holiday weekends such as Memorial Day).

Spring Term is the musical demonstration term for our Little Stars class. A demonstration of learning will take place in our classroom on Jun 1st with a class celebration on Jun 8th.

I, the undersigned, being the parent/legal guardian declare that the information on this form is correct to the best of my knowledge and I hereby apply for a place at the Spotlight Musical Theatre Academy (Spotlight) indicated for my child. I understand that Spotlight reserves the right to restrict admission at its own discretion.
CANCELLATION POLICY- I understand that once accepted by Spotlight I must give thirty (30) days notice, prior to the camp or term start date, of my child’s intention to cancel. If no or inadequate notice is given, I agree to pay the camp or term fees in full. A processing fee of $35 will be deducted from any refund given.
PARTICIPATION AND MEDICAL EMERGENCIES- I grant permission for my child to participate in all activities and I understand that Spotlight provides no health insurance or medical coverage and that the signing of this form acknowledges my responsibility for payment of any medical treatment which may be required while my child is participating in classes online or at the school. I further grant permission for Spotlight or its representatives to procure any and all necessary medical help for my child while they are under the supervision of the Spotlight school and authorize Spotlight or its representatives to permit any competent medical person to take all reasonable measures to treat any injury or sickness that my child may suffer.
RELEASE- By signing this form I hereby state that I release all members of the Spotlight staff and any other party involved in the organization and administration of Spotlight from any liability as a result of any injury or illness sustained in or around or online at a Spotlight venue.
PHOTO & VIDEO /AUDIO RELEASE-I grant permission for any and all photographs and video/audio recordings taken at Spotlight schools to be used by Spotlight or its representatives in displays or for advertising in, but not limited to, newspapers, magazines and brochures or on social media. I understand that no compensation is offered. I also understand that all photographs, video/audio recordings taken by Spotlight are the property of Spotlight.
